Do I need a referral to book a consultation at SLEEPMEDIK?

No referral is needed. If you are using insurance, check with your provider, as some may request a referral for reimbursement.

We provide care for patients 16 years of age or over only and do not offer services for children.

Does Southern Cross Health Insurance cover my consultation or sleep study at SLEEPMEDIK?

Yes, we are an Affiliated Provider. This means we can submit claims directly to Southern Cross for eligible services.

Coverage depends on your specific plan, so please contact your insurer to clarify what is included.

What should I expect during my first consultation?

You will receive a comprehensive review of your sleep symptoms, medical history, and lifestyle. We will explore possible diagnoses and recommend next steps, which may include a sleep study or a tailored treatment plan.

Which questionnaires should I fill in and bring to my appointment?

We recommend completing a few short questionnaires before your visit to help us better understand your sleep concerns:

  • Epworth Sleepiness Questionnaire – Epworth Sleepiness Scale – Recommended for all patients, no matter the sleep issue.
  • Sleep Apnoea Screening – Sleep Apnoea Screening (STOP-Bang Questionnaire) – Useful if sleep apnoea is suspected (e.g., snoring, pauses in breathing, daytime fatigue).
  • Insomnia Questionnaire – Insomnia Severity Index – If you struggle to fall asleep or stay asleep.
  • Sleep Diary – Useful for tracking sleep patterns – especially if you have difficulty sleeping, irregular sleep times, or suspected body clock problems.
  • Restless Legs Questionnaire – If you experience uncomfortable leg sensations or often feel unrested.
  • Anxiety and Depression Questionnaire – Hospital Anxiety and Depression Scale (HADS) – If you are feeling low, lacking energy, overly tired, or sleepy.
